GDPR Privacy Statement and Forms

For ELJGL’s Privacy statement see here. The following forms can be printed off and returned. Or they are downloadable as PDF files, and these can be completed on line and signed. All forms should be returned to djjwarren@gmail.com

The email with these forms attached, was sent out on the 5th May 2018, with this text:-

Dear Players, Parents/Guardians, Junior Convenors etc.
 
The GENERAL DATA PROTECTION REGULATION (GDPR) comes into force on the 25th May 2018. The EAST LOTHIAN JUNIOR GOLF LEAGUE  (ELJGL) is obliged to formalise the retention of any personal data it holds about you which it has acquired, or has been given voluntarily to us in the past. We also seek your permission to retain and/or add to that data where it is necessary to correct our records and to keep them current. This is vital for the future functioning of the League.
 
Attached are 3 different forms and we respectfully ask you to read only the appropriate forms, and complete them, and return them to me asap but no later than the 19th May 2018 please. They are in “Word” format so should appear to you as a “docx” file.  
 
Form 1 – For Players currently under 18 years old, or who already have, or will, become 18 years old in 2018, I respectfully ask that your “Parents and Guardians also complete and sign this form. 
Form 2 – For Players who became 18 years old on or before 31/12/2017, please complete and sign this form. Parents/Guardians details are optional on this form.
Form 3 – For “Associates” – i.e. Junior Convenors, Volunteers, or Parents/Guardians not included in Forms 1 or 2 above,  and anyone else who may assist in the functioning of the League, please complete and sign this form.
 
Parents/Guardians, please see above, and if you have more than 1 Junior Golfer in your family, please complete and sign the appropriate form for each Player, 
 
We have published our Privacy Statement on our website here, which outlines why the League needs your data in order to function, and how it handles your data etc, etc. It also outlines your rights and how to exercise them. Copies of these forms also appear there.
 
Please add my email address, djjwarren@gmail.com  and eljgleague@gmail.com to your “Contacts” to ensure that any communications we send to you in future reach you. And please save my phone numbers on your phones as well.
 
We would really appreciate your assistance in this matter as we can only continue to provide competition and development opportunities for our Junior Golfers if we can contact you and them. Our website and e-mails are the main sources of any information you may require.
